Illinois Mutual’s teleunderwriting is convenient for you and your client. Thoroughly fill out Parts A & C of an application along with any required forms* (available on the Agent Forum) with your client and then connect your client by phone with ExamOne to complete a secure fact-finding interview for Part B. Point-of-sale calls are welcomed where you can call ExamOne directly and pass the phone to your client to answer questions, or if preferred, we can contact ExamOne when we receive the application and have them contact your clients at their earliest convenience! They can even provide Spanish translation if necessary.
Personal information required for ExamOne’s interview includes:
• Medical history
• Names, addresses and phone numbers of consulting physicians
• Names of prescription medications
• Employment history
• Participation in various hobbies
• Driving record and license number
• Current job duties
