| It’s as simple as 1-2-3. Here’s how it works*: |
1. You introduce the online portion of the process:
- Submit EZ-App drop ticket with the customer’s email address
- Prompt your customer to use the email link that is sent to them

2. Your customer accesses the link from the email:
- Registers and logs in
- Completes
- Electronic Consent & Honesty Statement
- HIPAA authorization
- High level medical questions
- Upload Identification (optional)
- Submits it to TeleLife
|

3. TeleLife representatives:
- Complete the remainder of the interview via phone call
*Note: You can opt out of partial online completion during the EZ-App submission process if your customer prefers a full phone interview. |
